Fillable Fee or Roster Designation Application for Fee Personnel Designation (form VA 26-6681)
You may either click on the link, and fill the form out, or right-click the link
and select "Save Target As..." and save the form to your hard drive.
- If you fill the form out on-line, once the form is filled out, you need to save it first, by
clicking on the "floppy" icon on the toolbar. Once it's saved, please send to:
- A credit bureau report will be pulled on all applicants. The credit bureaus used are Trans Union, Experian, and Equifax.
Should you want confirmation that VA received your application, you must configure your e-mail software program to do so.
